§ Research
Tirzepatide is a synthetic 39-residue analog built on the native GIP sequence and engineered to activate both the GIP receptor (GIPR) and the GLP-1 receptor. First described by Eli Lilly researchers under the code LY3298176, it carries a C-20 fatty diacid moiety attached through a hydrophilic linker, which supports reversible albumin binding and a systemic half-life of roughly five days. It is approved for clinical use in several jurisdictions and serves as the reference compound for dual incretin receptor agonism in metabolic research.
Tirzepatide acts as an imbalanced dual agonist: its potency at the GIP receptor approximates that of native GIP, while its potency at the GLP-1 receptor is lower than that of native GLP-1. At the GLP-1 receptor it shows biased signaling, favoring cAMP generation over beta-arrestin recruitment, which is associated with reduced receptor internalization. Combined receptor engagement stimulates glucose-dependent insulin secretion, modulates glucagon release, delays gastric emptying, and acts on central pathways involved in appetite regulation. GIP receptor activity in adipose tissue has also been proposed to contribute to lipid buffering and insulin sensitivity.
Preclinical discovery work characterized balanced receptor pharmacology in cell-based assays and glucose-lowering and body weight endpoints in rodent models, followed by phase 1 proof-of-concept data.
The SURPASS clinical program characterized effects on glycated hemoglobin and body weight in participants with type 2 diabetes across single-agent and add-on study designs.
A head-to-head phase 3 trial (SURPASS-2) compared the compound with semaglutide in participants with type 2 diabetes, characterizing glycemic and body weight endpoints.
Mechanistic clinical studies have examined insulin sensitivity, beta-cell function, and energy intake, alongside pharmacokinetic data documenting a half-life of approximately five days.
