Overview

Testosterone cypionate is the 17-beta-cyclopentylpropionate ester of testosterone, the principal endogenous androgen. Esterification at the 17-beta hydroxyl increases lipophilicity, so that the compound formulated in oil forms a depot from which it is slowly released and then hydrolyzed by esterases to free testosterone. It is approved for clinical use in several jurisdictions and is a controlled substance in many. It is a standard reference compound in androgen pharmacology and pharmacokinetic research. The cyclopentylpropionate side chain is cleaved before receptor binding, so its pharmacodynamics are those of testosterone itself, while the ester governs absorption rate and the duration of serum exposure.

Mechanism of action

Following ester hydrolysis, free testosterone binds the androgen receptor, a ligand-activated nuclear receptor. Ligand binding induces dissociation from heat-shock proteins, receptor dimerization, nuclear translocation, and binding to androgen response elements, regulating transcription of target genes in muscle, bone, prostate, and other tissues. Testosterone is converted by 5-alpha-reductase to dihydrotestosterone, a higher-affinity androgen receptor ligand, and by aromatase to estradiol, which acts through estrogen receptors. Exogenous androgen exposure suppresses hypothalamic GnRH and pituitary LH and FSH secretion through negative feedback.

Key research findings

  1. 01

Population pharmacokinetic and pharmacodynamic modeling in healthy male subjects (2018) characterized depot absorption, serum testosterone exposure, and associated hormonal responses.

  2. 02

Comparative clinical pharmacology studies (1980) found broadly similar serum testosterone, dihydrotestosterone, LH, and FSH profiles following the cypionate and enanthate esters.

  3. 03

Clinical studies document dose-dependent suppression of gonadotropins and spermatogenesis, characterizing the compound's effects on the hypothalamic-pituitary-gonadal axis.

  4. 04

Preclinical and clinical studies characterize androgen-receptor-mediated effects on lean mass, hematocrit, and lipid parameters.
