Overview

Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor with a tetracyclic beta-carboline-derived structure that is chemically distinct from the pyrazolopyrimidinone scaffold of sildenafil. It is approved for clinical use in several jurisdictions. Its pharmacokinetic profile is distinguished by a long elimination half-life relative to other PDE5 inhibitors, and it is used as a reference compound in phosphodiesterase pharmacology and cyclic nucleotide signaling research. Its rigid ring system contributes to a binding orientation within the PDE5 catalytic pocket that differs from earlier inhibitors.

Mechanism of action

Competitively inhibits PDE5, the enzyme responsible for hydrolysis of cyclic guanosine monophosphate (cGMP) in vascular and visceral smooth muscle and platelets. Inhibition preserves cGMP generated by nitric oxide-stimulated soluble guanylate cyclase, increasing protein kinase G activity, lowering intracellular calcium, and producing smooth muscle relaxation. Tadalafil exhibits high selectivity for PDE5 over PDE6, the retinal isoform, while showing measurable inhibition of PDE11, which is expressed in skeletal muscle, testis, and prostate. It is metabolized predominantly by hepatic CYP3A4 to a catechol metabolite that is subsequently methylated and glucuronidated, with metabolites considered pharmacologically inactive.

Key research findings

  1. 01

Clinical pharmacokinetic studies in healthy subjects (2006) document a mean terminal half-life of approximately 17.5 hours, with linear exposure and no meaningful food effect on absorption.

  2. 02

Site-directed mutagenesis and binding studies (2012) identified specific amino acid residues in the catalytic domain responsible for tadalafil's selectivity for PDE5 over PDE6.

  3. 03

In vitro enzyme assays characterize its selectivity profile across phosphodiesterase families, including relative PDE11 inhibition.

  4. 04

Preclinical and clinical studies have characterized effects on pulmonary vascular resistance and lower urinary tract smooth muscle tone.
