Overview

Mazdutide (LY3305677, also designated IBI362) is a synthetic analog of oxyntomodulin, an endogenous hormone derived from proglucagon that activates both the GLP-1 receptor and the glucagon receptor. Originally discovered by Eli Lilly researchers and clinically developed by Innovent Biologics, the sequence carries a fatty acid side chain that supports albumin binding and an extended half-life. Mazdutide received its first regulatory approval in China in 2025 and is studied as a reference dual GLP-1 and glucagon receptor agonist in metabolic research.

Mechanism of action

Mazdutide activates the GLP-1 receptor and the glucagon receptor, both class B G-protein-coupled receptors that signal predominantly through cAMP. GLP-1 receptor activation stimulates glucose-dependent insulin secretion, suppresses glucagon release, delays gastric emptying, and engages central appetite circuits. Glucagon receptor activation, located mainly in the liver, is associated with increased hepatic fatty acid oxidation, reduced hepatic lipid accumulation, and increased energy expenditure. The insulinotropic effects of GLP-1 receptor agonism are proposed to counterbalance the glycemic effects of glucagon receptor activation.

Key research findings

  1. 01

Preclinical studies in rodent models of diet-induced metabolic dysfunction have characterized effects on hepatic lipid metabolism, endoplasmic reticulum stress markers, and inflammatory signaling.

  2. 02

A phase 2 randomized trial in Chinese adults with elevated body mass index characterized dose-dependent body weight, waist circumference, and metabolic biomarker endpoints against placebo.

  3. 03

Phase 3 trials (GLORY and DREAMS programs) characterized body weight and glycemic endpoints in participants with and without type 2 diabetes, including comparisons with dulaglutide.

  4. 04

Across clinical studies, the most commonly reported adverse events were gastrointestinal, with modest increases in resting heart rate also documented in several trials.
