Overview
CagriSema is a fixed co-formulation of two long-acting agonists developed by Novo Nordisk: cagrilintide, an acylated amylin analog that activates amylin and calcitonin receptors, and semaglutide, a GLP-1 analog with a C-18 fatty diacid side chain. Both components bind albumin and have half-lives of approximately one week, allowing them to be combined in a single formulation. The combination is investigated as a model of dual amylin and GLP-1 receptor agonism in metabolic research.
Mechanism of action
The two components act through distinct receptor systems. Cagrilintide activates amylin receptors (calcitonin receptor and receptor activity-modifying protein heterodimers) in the area postrema and hypothalamus, which is associated with enhanced satiation, slowed gastric emptying, and suppression of postprandial glucagon. Semaglutide activates the GLP-1 receptor in pancreatic beta cells, the gastrointestinal tract, and the central nervous system, stimulating glucose-dependent insulin secretion, suppressing glucagon, and modulating appetite circuits. Engagement of both pathways is proposed to produce additive effects on energy intake.
Key research findings
- 01
Each component has its own published literature: cagrilintide as a long-acting amylin analog and semaglutide as a reference GLP-1 receptor agonist.
- 02
A phase 1b study characterized safety, tolerability, and pharmacokinetics when cagrilintide was co-administered with semaglutide, with no clinically relevant pharmacokinetic interaction between the components.
- 03
A phase 2 trial in participants with type 2 diabetes characterized glycated hemoglobin and body weight endpoints for the combination relative to each component alone.
- 04
The phase 3 REDEFINE program characterized body weight and cardiometabolic endpoints in participants with and without type 2 diabetes, with gastrointestinal events as the most frequently reported adverse events.
