Overview
ARA-290 (cibinetide) is a synthetic linear 11-residue sequence modeled on the aqueous face of helix B of erythropoietin. It was designed to retain the tissue-protective signaling of erythropoietin while lacking its erythropoietic activity, avoiding effects on hematocrit and thrombosis risk. It was developed by Araim Pharmaceuticals as an investigational agent, and it has progressed through several phase 2 clinical studies. It serves as a reference agonist in research on the innate repair receptor, small-fiber neuropathy, and inflammation.
Mechanism of action
ARA-290 binds the innate repair receptor, a heteromeric complex of the erythropoietin receptor and the beta common receptor (CD131), rather than the classical erythropoietin receptor homodimer that drives red cell production. Receptor engagement activates JAK2 and downstream survival signaling and is associated with reduced pro-inflammatory cytokine production by macrophages and other innate immune cells. Studies in neuropathic pain models also report modulation of TRPV1 channel activity. Its plasma half-life is short, on the order of minutes, but downstream effects in preclinical and clinical studies have been reported to persist well beyond systemic exposure, consistent with initiation of a longer-lived signaling cascade.
Key research findings
- 01
A randomized, double-blind pilot study characterized effects on small-fiber neuropathy symptoms in participants with sarcoidosis.
- 02
Clinical studies using corneal confocal microscopy have characterized changes in corneal nerve fiber abundance in sarcoidosis-associated small-fiber loss.
- 03
Preclinical models of experimental colitis, impaired wound repair in genetically diabetic mice, and islet transplantation have characterized effects on inflammatory and tissue-protective endpoints.
- 04
A phase 2 clinical trial examined cibinetide in diabetic macular edema, characterizing best-corrected visual acuity and central retinal thickness endpoints.
